Treatment Guideline and Explanation
Supportive:
- Keep the baby warm
- Diet: NPO
- Fluid and electrolytes: IV fluid according to weigh and age
- Respiratory support: Oxygen inhalation 3-4L/min (up to 8L/min) through head box
- If baby does not improve and develop severe breathing difficulty with falling SPO2, then keep the baby in CPAP and if required in mechanical ventilator
- Broad spectrum antibiotics: Ampicillin (dose: 100mg/kg/day in 2 divided doses) and Gentamicin (dose: 5mg/kg/day once daily) if sepsis is suspected.
- Monitor: Respiratory rate, heart rate, temperature, cyanosis, air entry, CRT, SPO2
Specific:
Administration of surfactant (4 ml/kg divided into 4 aliquots) through ET tube.
